What you will do:
1. Conduct comprehensive assessments of patients with complex rehabilitation needs.
2. Develop and implement individualised treatment plans to promote recovery and independence.
3. Use a variety of physiotherapy techniques such as exercise therapy, manual therapy, electrotherapy, and mobility training.
4. Educate patients, families, and caregivers on treatment strategies and home exercise programs.
5. Work within a multidisciplinary team including doctors, occupational therapists, nurses, and social workers.
6. Provide rehabilitation in various settings including hospitals, community centres, outpatient clinics, or patients’ homes.
7. Monitor and evaluate patient progress, adjusting treatment plans as needed.
8. Prevent hospital admissions by supporting early discharge and community-based rehab programs.
9. Maintain accurate and timely patient records in line with NHS and HCPC standards.
10. Adhere to infection control, health & safety, and safeguarding policies.
Requirements:
1. HCPC registration is required.
2. Previous experience in general rehab is essential.
