The post holder will be expected to undertake assessments, design care packages and procure services for individuals with a learning disability who have complex health needs. These include the CHC process and Section 117 aftercare. The post holder will be expected to carry a caseload of people with complex physical and mental health needs, care manage, review and monitor their packages of care at least annually unless earlier re-assessment is required. The post holder will be expected to have good knowledge of the Mental Capacity Act (MCA) and be able to undertake both MCA and Best Interest decision making. The post holder will be part of a multi-disciplinary team, contributing to the effective provision of services to people with a learning disability with complex needs across the MDT ensuring that other teams in wider LD services are supported when needed. All staff working within the Learning Disability service are responsible for providing and maintaining high quality supportive relationships which at all times acknowledge the individual rights and personal dignity of the service user, encouraging individual opportunity for growth, physically, mentally and emotionally. In the performance of all duties and responsibilities the employee will be expected to show an in depth range of clinical experience and expertise in the field of Learning Disabilities coupled with an awareness of the entitlements of the service user.
