An exciting opportunity has arisen to join an dynamic forward thinking Discharge and Transfer Team. We are looking for enthusiastic and motivated team players with excellent communication, problem solving and organisational skills.

You will work alongside the Discharge and Transfer leads and ward teams to provide support and advice on discharge.
This role includes completing referrals to other services, working to tight deadlines and communicating with patients, families and the wider MDT.
Being a point of contact with both the ward team, patients and families is vital to help reduce the length of stay for patients.
• To provide support in order to facilitate and drive the safe, effective and timely transfer of patient care
from the Trust to the most appropriate setting to meet the needs of the patient.
• To take active steps to reduce the number of patients whose further care is delayed
• Liaise with Community Care Coordinators, Primary care, Registered Nurses, Doctors, Therapy Services, Home to Assess Team and any other members of the multidisciplinary team to provide a proactive and continuous process of discharge planning.
• Assist with training and support to the multi- disciplinary team in relation to updating the Ward Interactive Whiteboard system
• Act as the main point of liaison between Ambulance providers and the Trust
