Key Responsibilities Clinical Practice Identify, assess and diagnose patients with complex health needs across the practice population working to protocol and in close liaison with doctors Responsible for assessing, diagnosing and treating patients over 5 years old presenting with respiratory and ear nose and throat symptoms Undertake clinical reviews for mental health and dementia patients Triage and treat patients wishing to see a health care professional Lead the introduction and use of technical and specialist nursing treatments for patients with complex healthcare needs working to protocol and in close liaison with doctors Prescribe products to meet the needs of patients within the practice population Lead and develop programmes to improve health and well-being and meet the specific needs of communities, groups and individuals Work with others to change policies and improve health and well-being and reduce inequalities Lead the implementation and evaluation of evidence-based practice to improve the services offered by the practice nursing team Lead the team in quality assurance, development and clinical governance. Review, implement and evaluate individual treatment plans for patients with a known long-term condition as part of an annual review process Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Undertake relevant diagnostic tests within scope of Practice and competency Provide information and advice on prescribed or over-the-counter medication on medication, side effects and interactions Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF Request and undertake pathology services as necessary Process and interpret pathology and other test results as required Plan, implement and participate in vaccination and immunisation programmes for both adults and children Clinical Leadership Lead Nurse Prescriber Role within the practice Enable the practice nursing team to apply and improve clinical governance and risk management systems Development and implementation of policies, standards and guidelines across the practice nursing team, in close liaison with the doctors Conduct professional supervision and support with reviews and annual appraisals as requested Analyse, interpret and report data on health and well-being, related needs and service use Ensure agreed practice audits are carried out Ensure the Business Partner is aware of matter arising or problems/potential problems Develop a working environment that promotes healthy, safe and effective work outcomes Monitor and evaluate patient care processes and outcomes, ensuring that patient and practice needs are met efficiently and effectively Lead and manage quality improvement projects aimed at improving patient services See attached job description for full job description including the following areas: Communication Delivering a Quality Service Team Working Management of Risk Equality and Diversity Confidentiality: Health & Safety: Personal/Professional Development: This job description is an outline of the post and is not exhaustive.
