The Community Neuro and Stroke Team are a well-established team, delivering highly specialist care to individuals following neurological injury, diagnosis, and stroke. The team comprises highly experienced Physiotherapists, Occupational Therapists, Speech & Language Therapists, Nurses, Dietitians, Clinical Psychologists, and a forward-thinking consultant in Neuro rehab, all keen to share their knowledge and experience to enhance care for people in the Wigan borough.
We are seeking an Occupational Therapist who has a keen interest in neurological rehabilitation and would like to develop their knowledge and skills in this area. You will work closely alongside and be supported by other Specialist staff in the team.
This is a permanent part-time post (30 hours) worked Monday to Friday with some Saturdays but no bank holidays.
An Enhanced DBS with Adults' Barred List check will be obtained during pre-employment checks.

Main duties of the job
* Provide a high standard of intervention to patients with neurological dysfunction within the interdisciplinary Community Neuro and Stroke Team in various settings across Ashton, Wigan, and Leigh.
* Act as a source of knowledge for patients presenting with a range of neurological conditions, providing advisory/educational services to patients, relatives, therapists, and other healthcare professionals.
* Participate in the specialist assessment and treatment of patients with complex and chronic presentations, determining clinical diagnosis and treatment indicated.
* Manage a defined caseload using evidence-based/client-centered principles to assess, plan, implement, and evaluate interventions.
* Provide leadership and clinical supervision for junior staff as delegated by the manager and/or professional supervisor.
* Collaborate within the ICS model in Wigan and liaise with professionals and voluntary agencies to optimize patient care.

Duties and Responsibilities
• Be professionally and legally responsible and accountable for all aspects of your work, including the management of patients in your care.
• Interpret and analyze clinical and non-clinical facts to form accurate diagnoses and prognoses in a wide range of conditions.
• Ensure interventions take place in the most appropriate setting, e.g., hospital or clients’ homes.
• Work with clients and/or carers to identify goals as part of an overall care plan in cooperation with the inter-disciplinary professional team.
• Demonstrate and apply an understanding of the effect of disability and long-term condition management, ensuring high standards of holistic clinical care.
• Provide advice, teaching, and instruction to relatives, carers, and other professionals to promote understanding of intervention aims.
• Provide advice and support to colleagues regarding patients with neurological dysfunction.

Planning and Organisational Duties
• Plan, prepare, and undertake client assessments for a designated caseload in line with professional and national standards.
• Use clinical judgment to prioritize caseload in line with clinical guidelines.
• Plan and implement individual and/or group interventions in collaboration with clients to achieve therapeutic goals.
• Monitor, evaluate, and modify treatment for clients with multiple needs/pathology.
• Ensure appropriate selection of treatment modalities and sites for delivery of interventions.
• Maintain accurate and comprehensive patient treatment records in line with Trust Policies Practice.
• Justify clinical judgments when differences of opinion occur and demonstrate understanding of Clinical Governance and Risk Management.

• Supervise and evaluate students and junior staff as delegated by senior staff.
