We are looking for an exceptional individual to join the Transfer of Care Team at the Royal London Hospital as a Lead Screener and Discharge Navigator. The Transfer of Care Team is a joint health and social care service aimed at facilitating discharges from acute settings, based on the home first principle. The team prevents avoidable delays in patient discharges through coordination and planning in accordance with current government discharge guidelines. You will problem-solve to overcome challenges and work with the ward multidisciplinary teams, senior management and external agencies as well as advocating on behalf of your patients.
You will be able to manage conflicting priorities, be confident in challenging decisions at all levels and have experience of working in a high paced, high pressure environment. You will supervise more junior staff and will yourself be offered senior support and training to help develop you in your role.
We are a 7 day service operating from 08:00 to 20:00 including bank holidays. If successful, please note you will be expected to work some rota'd weekends, bank holidays and late shifts. We are open to flexible working patterns.
Main duties of the job
1. To lead the discharge assessment and planning of patients with complex discharge needs, from advanced planning prior to discharge to the point of onward transfer, with a proactive case management approach drawing on post holders specialist physiotherapy professional background.
2. To lead on co-ordinating early complex discharges so patients leave hospital within 24 hrs of being declared Medically optimized and ready for discharge in order to minimise discharge delays, improve inpatient flow and reduce the possibility of clinical deconditioning.
3. To proactively identify and escalate any potential delays in discharge and lead on complex problem solving/case review meetings requiring analysis and interpretation of a number of complex factors and options as required.
4. To lead on screening referrals daily, identifying the appropriate pathway for discharge for community health and social care services based on assessment, analysis and reasoning through an evidence-based approach.
5. To promote safe and effective discharge practice throughout the Trust and act as a resource for expert for teaching, advice and guidance as required.
6. As a senior specialist in this field you will supervise and line manage more junior staff, supporting them through teaching and guidance with their more complex caseload management.

Experience
* Experience of working in acute/tertiary hospital setting.
* Experience of discharge planning and liaison work.
* Involvement with clinical networks/patient pathways within acute hospital setting.

* Evidence of working at a strategic level.
Qualifications
* Relevant Degree/Diploma, registered senior therapist.
