Job summary An exciting opportunity has arisen to recruit a Patient Care Co-ordinator to the Urology Team. The successful applicant will be actively involved in providing direct and indirect care for patients in support of the multidisciplinary team. The post holder will be expected to work under direct or indirect supervision of the clinical nurse specialists supporting the cancer multidisciplinary team. This role will include providing cover for other members of the team for the Acute Urology Cancer pathway when required. Will provide a single point of contact for the patient/carers and will be expected to work autonomously to provide this service. Will co-ordinate with the multi-disciplinary team to facilitate safe and effective transition of care between services and neighbouring trust in order to provide seamless service.
