Job summary An exciting opportunity has arisen for a highly motivated and enthusiastic individual to join our dynamic and compassionate team at Singleton Hospital, which hosts the South West Wales Cancer Centre. Our leading Cancer Centre specialises in providing comprehensive, patient-centred inpatient and outpatient Systemic Anti-Cancer Therapy (SACT), radiotherapy services, and autologous stem cell transplant as part of the South Wales Blood and Marrow Transplant Programme. This role includes provision & development of an advanced pharmaceutical care service for oncology patients. It will enable you gain experience within breast oncology treatment clinics, reviewing, monitoring and prescribing (if able) SACT treatments and supportive care. You will have the opportunity to integrate into the wider breast oncology multidisciplinary team working closely with a variety of healthcare professionals to provide patient care. Taking part in weekly MDT meetings and frequent clinical discussion meetings. Enabling opportunities for service improvement and development and ensuring practice is standardised, safe and up to date. Main duties of the job You will gain experience working in a patient focussed environment using your knowledge and skills to improve patient care and promote optimal outcomes from medicines, this can include undertaking research and/or audit. Provide expert advice and guidance on the safe and effective use of medicines, in addition to production of drug related policies and shared care protocols. Liaising with health professionals in other tertiary centres and in community care to provide on-going pharmaceutical care and advice when patients transfer between settings, ensuring continuity of medication supplies. You will also engage in clinical screening and provision of SACT to patients both via hospital sites and through homecare facilitators. This can include liaising with external providers to arrange PO/ IV/ SC/ IM SACT administration for patients at home. There is a strong focus on learning and development, with the post facilitating an education & clinical leadership role within Cancer services, supporting not only patients, but junior pharmacists and other health professionals/undergraduates. Responsibilities also include investigation of medication related incidents or complaints within oncology services and exploration into ways of minimising the risk of future re-occurrences.
