To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The practice nurse will work in collaboration with the nursing team in a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health prevention, well women clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.
We are looking for 24 hours per week, between the hours of 8:00am and 6:00pm, Monday to Friday.
Main duties of the job
1. To provide and maintain a high standard of care to our patients.
2. To support and provide assistance to the GPs and other members of the team.
3. To be responsible for the care & management of delivering nursing services to our patients.
Job responsibilities
The following are the core responsibilities of the practice nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
1. To develop, implement and embed health promotion and well-being programmes
2. To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
3. To provide wound care (ulcer/Doppler etc.) to patients
4. To request pathology services as necessary
5. To process pathology results as required
6. To maintain accurate clinical records in conjunction with extant legislation
7. To ensure read codes are used effectively
8. To develop, implement and embed well woman clinics
9. To chaperone patients where necessary
10. To prioritise health issues and intervene appropriately
11. To support the team in dealing with clinical emergencies
12. To recognise, assess and refer patients presenting with mental health needs
13. Ensure infection control guidelines are maintained
14. To implement vaccination programmes for adults and children
15. To support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
16. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
17. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
18. To support the clinical team with all safeguarding matters in accordance with local and national policies
19. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
20. To deliver opportunistic health promotion where appropriate
