Job responsibilities
Responsibilities and Duties of the Job:
lTo produce and comply with a work plan developed in partnershipwith the Practice Manager and Partners to ensure all Practice and other work is delivered appropriately and safely
lTo undertake appropriate clinical risk assessments and work with the wider teamto implement management strategies for risksidentified
lParticipate in activities required to assesscompliance with CQC FundamentalStandards
lTo provide high-quality medical services in the contextof general practiceto the Patients registeredwith The Welby Group
lTo provide a full range of servicesin line with the agreedrequirements of the practice NHS contract includingthe quality and outcomes frameworkand any local and enhanced services
lTo provide any services as required for the PrimaryCare Network agreement including shared QOF standardsand service specifications.
lTo provide appropriate care and servicesto Patients in all age ranges and levelsof health care needs
l To respondto medical/health problemspresented by Patientson the registered
list including history taking, diagnosis, investigation, treatment and referral as appropriate
lTo provide management of long-term conditions in line withnational guidelines and pathways
lTo read and interpret test requests and results orderedby you and other team members taking appropriate actionbased on the findings
lTo provide appropriate health promotion and preventative healthcare advice to allPatients attending the practice
lTo prescribe generically for Patients as appropriate to need and be able to provide appropriate clinicalrationale with supportfrom the Medicines Management Team
lTo carry out appropriate administrative and other duties in line with the needs of the Practice including referral toother services, maintenance of records, managing registers etc.
lTo maintain clinicalrecords either writtenor computer basedin line with professional code and requirements utilising appropriate coding
lTo work in line with appropriate Practicepolicies and procedures including humanresources, safeguarding children etc.
lTo provide appropriate health education and advice to Patients and carers to enable self-management and choice
lTo undertake appropriate audit in practiceto identify clinicalquality issues and markers and define processes toimprove clinical outcomes
lTo take part in appropriate R&D and audit processes withinThe Welby Groupas agreed in line with primary care division priorities
l Assist management with maintaining compliance with agreed standards
l Promote and implement all the Practicepolicies and procedures
lIn the courseof your employment, you will have access to confidential information relating to The Welby Groupand Patients. You are required to exercise due consideration in processing suchinformation and you should maintain confidentiality for The Welby Group and Patients
lThe post holder will, if required to do so, obtain, processand/or use information in line with the GDPR requirements
