About the role Join our Wellbeing Partnership Integrated Care Team as a Referral Coordinator The Wellbeing Partnership consists of multi-agency health and social care practitioners, who aim to provide coordinated care and support to enable people to stay well in their own community. As part of your role you will be working proactively with the practitioners in the partnership to promote the ethos of integrated care. Some of your duties will include: Acting as first point of contact to practitioners and members of the public Taking a lead role in the coordination of referrals into and out of the partnership, collating information and presenting to partnership meetings if necessary Providing telephone support, offering advice and information regarding alternative community resources when necessary Undertaking basic assessments to identify individual s needs, which will include assessing levels of risk and may include visits to people s homes and/or other services Facilitating team meetings, preparing agendas, updating records/notes on electronic records during the meeting and disseminating information as necessary If you are an enthusiastic and compassionate individual who is committed to ensuring high standards of care and support will be delivered to our service users at all times, we would love to hear from you A workplace that invests in you A competitive salary that increases each 12 months until you reach the salary maximum.
