The Virtual Ward has been commissioned to provide a safe alternative to manage and monitor a specific group of patients with acute respiratory illness, Frailty or Heart Failure as an alternative to hospital admission and/or to facilitate earlier discharge from hospital. The patients' care will be delivered in their own home by utilising cutting-edge patient-friendly technology to provide remote monitoring, along with additional tailored in-person or telephone support.
The role of the virtual ward team is to coordinate the hospital-based and community-based elements of the ward by:
* Promoting the benefits of the virtual ward to patients and HCP
* Identifying and admitting patients to the virtual ward, via step up/step down pathways
* Assessment and remote monitoring of patients in the community
* Delivering specialist expertise to ensure patients feel supported on the virtual ward
* Employing holistic assessment and strategies to manage emerging health needs.
* Providing administrative duties to facilitate the smooth operation of the service
* Integrating into the wider Respiratory/Frailty/cardiac directorate.
Main duties of the job
As a Virtual Ward Staff Nurse you will:
* Participate in assessment and care planning of patient care in conjunction with the clinical nurse specialist/AHP on the virtual wards.
* Implement and evaluate care and treatment in agreement with the patient and as defined in the care plan, escalating any deviation from the care plan or change in condition to the appropriate senior decision maker accordingly.
* Communicate effectively with patients, carers, and all members of the multidisciplinary team.
* Prioritise daily work in line with clinical/patient needs.
* Maintain accurate and legible patient records in line with professional and organisational requirements; this includes the electronic patient record system.
* Implement and work to ensure evidence-based care/best practice.
* Actively participate and contribute to team meetings and daily handover.

Qualifications
* Level 1 Registered Nursing Qualification
* Evidence of continuing professional development
* Demonstrable recent Nursing experience.
* Post registration qualification in chronic disease management.
* Experience of acute and community working
Knowledge and Experience
* Ability to use initiative, prioritise effectively

* Demonstrable understanding of the scope and extent of the role.
