1. Job Purpose
Assist practice GPs/Nursing team and other Health Care Professionals to deliver effective and holistic management of patients with chronic diseases as required by the Quality and Outcomes framework (QOF).
1. Contribute to the development of policies / protocols / guidelines within the practice.
2. Aid the practice team with implementation of new initiatives in the delivery of chronic disease management/enhanced services.
3. Support the implementation of evidence-based guidelines in conjunction with the practice clinical team.
4. Encourage the development of integrated services within Primary Care to include the practice-based teams and multidisciplinary working with external providers.
2. Job Summary
The post holder is an experienced nurse, who is responsible for ensuring the delivery of safe and effective patient care to the practice population using autonomous clinical skills and a broad in-depth theoretical knowledge base.
1. You will manage a diverse clinical case load, dealing with the presenting patient’s needs.
2. You will contribute to developing the nursing services offered by the practice.
3. You will work independently and contribute to audit, supporting, implementing and evaluating local standards, policies, and guidelines when required.
4. NMC requirements will be met, and the individual will work within the NMC Code of Conduct to agreed practice standards and protocols.

1. You will be accountable for your own practice, working independently managing your clinical workload whilst responding effectively to patient needs, in a timely manner.
2. You will mentor and support junior staff with development and maintaining skills.
3. To provide input and advice on evidence-based practice and care interventions within their sphere of knowledge.
4. Clinical Role
1. Assess, plan, develop, implement and evaluate treatment programmes for patients that promote health and well-being.
2. Help the practice develop and deliver specialist primary nurse led services, including chronic disease management (including for all areas of QOF).
3. Manage programmes of care for patients with chronic diseases by planning, providing and evaluating care under agreed guidelines and patient group directions.
4. Advise members of the public on the concepts of health promotion, education, prevention and protection.
5. Undertake annual reviews of patients understanding and ability to self-manage.
6. Make professionally autonomous decisions for which you are accountable and provide safe, evidence based, cost effective, individualised patient care.
7. Where necessary evaluate patient test results in conjunction with the patient and their GP and ensure the appropriate course of treatment is initiated.
8. Make appropriate referrals to other members of the primary, community and secondary care teams.
9. Administer child and adult immunisations and vaccinations in accordance with national and local programmes, ensuring safe storage, rotation and disposal.
10. Competent in anaphylaxis and resuscitation techniques.
11. Undertake procedures including venepuncture, glucose tolerance tests, blood pressure checks, urinalysis, peak flow, new patient health checks, pregnancy testing, stitch/clip removal, cervical smears, wound management including compression therapy.
12. Provide support and chaperoning to the doctors for minor surgery, coils and other procedures.
13. Contribute to achieving the practice targets.
14. Maintain clear, concise and accurate records in line with the Nursing & Midwifery Guidelines.
15. Ensure all data protection requirements are met when gathering, recording and storing patient data.
16. Liaise and maintain good working relationships with all members of the practice team.
17. Work within the multi-disciplinary team within the practice and across the wider health system.
5. Professional Role
1. Attend and contribute to relevant internal and external meetings as required including practice clinical meetings, and nurse team meetings.
2. Keep up to date with current training and developments, identify own training needs and attending suitable courses appropriate for the job role.
3. Promote evidence-based practice through use of the latest research-based guidelines.
4. Monitor the effectiveness of their own clinical practice through quality assurance strategies such as the use of peer audit and review.
5. Maintain professional registration.
6. Work within the latest NMC Code of Professional Conduct.
7. Record accurate consultation data in patients records in accordance with NMC guidance and other standards.
8. Demonstrate Leadership.
9. Help the practice develop in a cost-effective manner, monitoring and using resources appropriately.
10. Participate in audits and inspections as required.
11. Promote effective communication, relationships and team working within the practice.
12. Facilitate and promote the use of up-to-date technology to develop practice.
13. Management of risk. This entails the risk assessment of all situations, taking appropriate actions and reporting of all incidents, near misses and hazards.
14. Health and Safety requirements: There is a statutory duty of care for one’s own personal safety and that of others who may be affected by their acts or omissions. Employees are required to co-operate with management to enable the practice to meet its legal obligations and to report any hazardous situations or defective equipment.
