The post holder will be responsible for managing the anticoagulant service, including staff members and patients, ensuring a co-ordinated approach to new and ongoing anticoagulation management. The role will involve operational working within clinics, both at the hospital site, in community-based settings, as well as domiciliary visits for patients who are on home testing. In addition to operational duties, the post holder will be required to manage rosters, including the authorisation of annual leave, undertake line management and pastoral support to the anticoagulant team, review and develop anticoagulant guidelines and protocols relating to the prescribing and ongoing management of anticoagulation and participate and facilitate audit data collection for submission to the Trust in relation to key performance indicators.Working for our organisation The Anticoagulant Team at East Lancashire Hospitals NHS Trust is a nursing-led unit, responsible for managing patients on long and short term anticoagulation within the East Lancashire and Blackburn with Darwen area. The post holder will be required to have a wide range of communication skills in order to effectively communicate with patients, staff and other members of the multi-disciplinary team. The role of working within the Anticoagulant Team provides a great deal of job satisfaction due to the combined autonomous and team working aspects and diversity of clinical settings on a day-to-day basis.Detailed job description and main responsibilities
* Provide a nurse-led clinic service to patients with conditions requiring long and short-term anticoagulation with vitamin-k antagonists.
* Review and develop nurse-led anticoagulant clinics in line with service user requirements and staffing, including a recruitment and selection strategy to maintain staffing positions in line with establishment and service need.
* Have an in-depth knowledge of adjunctive therapies that are appropriate to incorporate into individual anticoagulation treatment plans.
* Develop nurse prescribing protocols and Trust guidance for vitamin-k antagonists and low molecular weight heparins (LMWH).
* Ensure on-going assessment and treatment of patients meets the current evidence-base as per NICE guidelines.
* Work within agreed local Trust and national policies, procedures, and guidelines, including those relating to patient consent and confidentiality.
* Acknowledge limitations in knowledge, referring to senior medical staff for further advice and clinical input when scope of practice is exceeded.
* Empower patients to enable them to become actively involved in the decision-making process regarding the management of their conditions requiring anticoagulation and ensure method of ongoing monitoring of anticoagulation is the most efficient for the patient as well as the service.
* Respond to cultural and/or social issues involved in chronic disease management and tailor patient plans around these.
* Work closely with other staff groups within the Trust and wider ICB to improve the discharge process for inpatients receiving vitamin-k antagonists.
* Understand of how interventions from other members of the multi-disciplinary team impact on the overall management of chronic disease relating anticoagulation.
* Ensure there is a follow-up process for patients who require changes in management of their anticoagulation in respect to acute interventions or treatments.
* Provide a high standard of care for patients with acute changes in disease activity and, if necessitated, arrange emergency treatment where appropriate.
* Work closely with the Trust PGD lead to develop new, and review existing, patient group directions (PGDs) to ensure the timely and effective supply and administration of medications to meet the patient needs.
* Understand the importance of networking within the wider anticoagulation community, across interfaces such as the ICB and neighbouring Trusts, to align the care patients receive in relation to their anticoagulation.
* Ensure the anticoagulant team respond timely and appropriately to anticoagulant referrals.
* Work autonomously while managing own caseload in nurse-led clinics, applying expert knowledge to make changes to a patients care plan.
1st level Registered Nurse with active NMC registration
* Advanced life support (ALS) qualification
