Main duties of the job The successful candidates will need to be able to demonstrate knowledge, skills and experience of person-centred assessments. The role will involve working within a multi-agency environment to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC) and S117 aftercare including the promotion and review of Personal Health Budgets. Experience undertaking mental capacity and best interests' assessments is advantageous and an understanding of deprivation of liberty safeguards and the Court of Protection would be beneficial to the role. You will also need to be knowledgeable in report writing to ensure you can confidently complete a health needs assessment, personal care and support plan and the CHC decision Support Tool. You will need to be competent in using Microsoft Word, Excel and Outlook and also be confident in learning new systems for recording and report writing. About us North Nottinghamshire CHC delivery team supports the facilitation of CHC services in Mansfield, Ashfield, Newark and Sherwood. Although the wider Nottinghamshire system for CHC services will be joining together from the 1st of April 2025. Staff working within the ICB are expected to have strong links with internal and external teams across this patch as well as the wider ICS. All individuals will have an identified base, but travel for assessments is expected as part of the role.
