EPiC brings together GP Out of Hours, Locality Clinical Assessment Service (LCAS), ED Streaming as well as our additional services of Enhanced Access and Same Day Urgent Care. Ensure that when referrals are made for patients to see other medical service providers, these are kept within local guidelines, and that adequate information is given within the referral to enable further assessment. Making every contact count. Front line staff are in an ideal position to offer support and advice on how to improve health and wellbeing. Staff will be given training and support to help them signpost people to other services. Maintain accurate and contemporaneous records to relevant professional standards/guidelines. Ensure patient notes are fit for purpose and information entered on the Patient Electronic Record is/stored in accordance with appropriate protocol. Support the provision of EPIC Services and promote the fulfilment of the rota. To monitor stock levels and ensure stock are ordered and maintained/controlled as required. Communications & leadership To attend departmental meetings as required. Ensure that effective communication with patients and staff is maintained. Provide reassurance to patients when providing care gaining consent by using empathy and persuasive skills. Ability to communicate with patients with learning/ language disabilities. Ability to communicate with challenging or confused patients. To highlight any areas of concern or improvement to line management.
