We are recruiting for a Personalised Care Support Worker/ Administrator who share/s our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.
You will be expected to be an experienced administrator who has excellent communication and IT skills to work with our Breast Cancer teams to implement Personalised care pathways for patients with Breast cancer.
Supporting forPersonalisedCare Follow Up (PCFU), you will form an integral part of the multidisciplinary team,alongside the CNScaring.You willassistthe MDT in coordinating all appointments, signposting patients as necessary to the relevant team member to support the patient through the self-managed part of their pathway.

Act as point of contact for queries relating to scheduling of patient appointments and co-ordination of patient follow uppathways.
Respond to patient and other Healthcare professional’s communication acting as a triage for patients and ensuring they are signposted to theappropriate memberof the team.
Manage thedatabasefor all patients on the PCFU, liaising with the MDT coordinator to ensure patients are reviewed as necessary at the weekly MDT.
Be aware of the physical,emotionaland psychological needs of the patient. Able toobserve, listen and report any changes to the patient’s condition to a member of the MDT so necessary guidance can be given. Organise the survivorship workshops including organising the agenda, patient attendance and relevant informationbeing available topatientson the day.
Plan andorganiseyour ownworkload, with guidance fromBreast Service Co-Ordinator.
Enter patient dataaccuratelyinto administrative systems ensuring this is undertaken in a timely manner.
Review cancelled/suspended/deferred patients ensuringappropriate actionis takenafter consultation with members of the MDT.
Ensure that all health records are tracked and securely stored according to departmental policy.
