The West Dumbarton Older Adult CMHT in Goldenhill Resource Centre, Clydebank is looking to recruit a Band 6 Care Home Liaison Nurse to their team.
The successful applicant will provide mental health nursing advice, support and education to identified care homes within West Dunbartonshire HSCP. They will work closely with Multi-Disciplinary colleagues within the Older Adults’ service and be responsible for assessing the mental health needs of clients who are referred from the care homes. They will liaise with local community services and work closely with the clients RMO (GP) to agree and co-ordinate care packages. They will be pivotal in providing assessment and diagnostic services to the care homes, providing ongoing care management, ensuring care plans are regularly reviewed with an aim to maintain the clients’ level of ability and prevent hospital admission. They will also link with the hospital discharge service to facilitate earlier discharges which follow the identified care pathway. The successful candidate will also be responsible for management of their own caseload and ensure effective communication between care homes and the local Community Older Adult Service.
