To complete appropriate risk management plans to minimise any risks to the service user and/or others and seek support if required. Undertake daily review of Service Users health and social care needs as required against the care plan, and take appropriate action to achieve the planned aims To support the documentation, monitoring and review of a range of specialist therapeutic interventions for service users To deliver holistic evidence based care to service users and their families using evidence based interventions and assessment tools, seeking advice/assistance from senior colleagues as required. To deliver evidence based daily therapeutic groups and daily one to one sessions, ensuring that these are planned collaboratively with the service users and the wider MDT To monitor, evaluate, and modify individual and/or group interventions in order to ensure effectiveness and to improve service user therapeutic outcomes. This requires observational skills, attention to detail and the ability to seek support/advice from senior colleagues as required. To liaise with the multi-disciplinary team, care coordinators, general practitioners and any other relevant parties involved, as per NICE guidelines and local policy. Advise service users, their family members and professional colleagues on appropriate lifestyle changes to an individuals physical and social environments which will promote enablement and independence. To participate in CPA reviews involving service users, their families and other professionals and agencies. To identify when a carers assessment is required and to ensure referral on to appropriate agencies and support networks (with support from senior colleagues). To act as an advocate for our service users and ensure that their voice is heard throughout their care To assist in maintaining the safe administration and storage of medication in accordance with Navigo policies and procedures. You will need to demonstrate the proficiencies for medication administration as set out in the NMC standards of proficiency and Navigo medicine management policy. Once signed off as competent by the Pharmacist and having successfully completed your annual medicines administration competency you will be expected to administer medicines in your working day Prompt, enable and encourage Service Users to be compliant with their medication regime and support junior staff to do this under the supervision and direction of the care coordinators. This may involve the use of concomitant aids, the collection of medication from community or hospital pharmacies and the delivering of medication to Service Users To complete medication audits/medication care plans/support in the assessment of individuals Under the direction of the service manager, you will be responsible for the day to day running of both Hope Court and Dudley Street flats. To plan and organise workloads for staff on shift - to include allocation of daily jobs, planning care and interventions such as group work and one to ones ensuring that all are working to their care plans. To review and reflect on own clinical practice and performance through effective use of professional and operational supervision and appraisal in line with local and professional guidelines. Demonstrate compassionate and inclusive leadership to ensure that all staff feel valued and effective and confident to speak out where needed. Support senior colleagues in the leadership of the service by speaking out and offering your opinion regarding service improvement. Support development and maintenance of links with other agencies and external colleagues. Attend peer supervision groups as and when required to do so. Required to deputise for the unit manager within the rehabilitation service for short periods of time. Undertakes responsibility for specific issues for example, quality assurance systems, Health and Safety and stock taking tasks. The post holder will be expected to mentor the work of junior staff and support care workers in their role, providing advice, supervision, training where appropriate in line with Navigo policy. To undertake the role of supervisor to nursing and Allied Health Professional students providing guidance, advice and written assessments and an evaluation of their performance when appropriate to do so. To be actively involved in the induction and training of newly appointed staff and visitors.
