To develop recovery plans using a person centred approach to develop, implement and evaluate needs and strengths based care plans and risk management plans in partnership with service users, carers and other professionals. To refer service users to appropriate Health and Social Care services that support their recovery and support them in accessing these services. To lead and participate in the clinical activities of the Team such as evening service and duty worker system. To make full use of all available interventions for delivering treatment, including online To work as an effective member of the multi-disciplinary team ensuring effective communication with other team members and other professionals. To attend, lead and contribute to multi-disciplinary and multi-agency clinical meetings. To receive and participate in clinical supervision in accordance with Trust policy To attend, lead and contribute to team meetings.
