We are looking to recruit Family Support Workers for our Children with Disabilities Team and Children's Transitions Team based at The Portal in Ellesmere Port.

The Children with Disabilities and Children’s Transitions Team are based in Ellesmere Port and are co-located with wider children’s social care, early help and education teams. The Children with Disabilities Team works with young people from birth up to age years up until they are ready to transition to our Children’s Transition Team that works with children aged - years. We work across the whole borough of Cheshire West and Chester so there will be a requirement to travel. The family support worker role is wide and varied and involves working directly with children and their families within their own homes and in the community. The provide evidence based interventions including parenting programmes and interventions to support young people with communication across all settings.
