The Children's Community Nursing Service has a part-time vacancy for a clinical coordinator role to work in our newly established Clinical Co-ordination hub, which operates 7 days a week, from 8.30 am to 5 pm, Monday to Friday, including some weekend cover. The coordinator will work alongside other coordinators and the management support officer (MSO) within the hub.
The aim of the coordination hub is to facilitate and coordinate clinical and operational care for children and young people accessing our service. The hub oversees the four clinical teams daily and is the first point of contact for referrers, patients, and clinicians.
We are seeking a Band 6 nurse with expertise in community nursing, who is an experienced clinician, motivated, and eager to explore and test new ideas and ways of working to improve service coordination.
Main duties of the job
* Work flexibly and proactively develop and test new ways of working in this developing role.
* Coordinate clinical care and allocate resources across the service daily.
* Facilitate daily huddles to ensure safe staffing of teams.
* Support staff sickness management and work reallocation.
* Triaging referrals to ensure timely and appropriate care.
* Handle clinical queries from referrers, clinicians, and staff, linking with caseload holders for safety.
* Possess skills and knowledge to cover a caseload as needed.
* Conduct difficult conversations with confidence.
* Update care plans and communicate key changes effectively.
* Oversee clinical waiting lists and assist teams in managing them.
* Participate in health panels, radar panels, and multiagency panels as a health expert.
* Provide clinical and child protection supervision to staff as needed.
* Maintain clinical competencies across all teams.
* Coordinate and deliver clinical training sessions for staff.
* Be familiar with or willing to learn SystemOne and Health Roster systems.
