This is an exciting opportunity to be part of a team putting people first to make sure they get the right support at the right time, ensuring same day discharge for those individuals who are clinically ready to return home. As part of our Home Assessment Team, you will work alongside Health & Social Care and Acute colleagues. The focus is on prevention of delays in hospital discharge arrangements, working alongside the individual and their families, and other support networks, to ensure good quality support in those first days after returning home.
We are looking for someone to lead, develop and motivate staff within the Home Assessment Team. You will have an excellent track record with significant post qualifying experience and should be highly motivated and an inspirational social work practitioner. You will be passionate and committed to driving forward change, whilst promoting innovation in a culture where people feel supported to do their jobs.
Minimum Requirements:
Significant post qualifying experience is essential together with a CQSW/DipSW, BA Hons in Social Work or equivalent. You are required to become a member of the Protecting Vulnerable Groups (PVG) Scheme prior to commencing in this role and be registered or eligible for registration with the Scottish Social Services Council (SSSC).

Work pattern:
35 hours per week - Monday to Thursday - 08:45 to 16:45 - Friday 08:45 to 16:15.
