The post holder provides expert clinical pharmaceutical advice on issues relating to medicines optimisation and use within primary care to all members of the practice teams (e.g. GPs, practice nurses, administration staff) and across the PCN neighbourhood footprint. The post holder manages the high quality, safe, cost-effective and evidence-based use of medication within the PCN neighbourhood footprint, including audit, training and education. Establishing and maintaining good working relationships within the multidisciplinary team is key to this role. As a member of the practice team/PCN neighbourhood footprint the postholder will be aware of the needs of patients and carers and will work within the appropriate level of competence. The work is of a highly confidential nature and no information, either medical or personal, should be disclosed outside of practice personnel.
