Main duties of the job
Thepost holder will be responsible for individual assessment and processing of allreferrals received at the hospice and will promote and monitor safe andeffective practice over a flexible 7-day rota. They will enhance the serviceuser experience and provide effective leadership/collaborative team working andcontribute to the delivery of the organizations objectives and strategy.
The Clinical Specialist role will work closely with a Referrals Hub Manager, ReferralsHub Clinical Nurse Specialist, Referrals Assistant and Referrals &Admissions Coordinator.
TheReferrals Hub team will work pro-actively across all service areas within SaintFrancis Hospice to ensure that all plans and arrangements following assessment arein place for a persons acceptance and admission to all hospice services

The Referrals Hub Clinical specialist will workpro-actively across all services areas within Saint Francis Hospice (SFH)ensuring that all processes are in place for an individuals referral to SFH Servicesand that access to services is appropriate and in real time. The post holderwill work closely with Community Services, Ward, Therapies /Individual & FamilySupport Services, OrangeLine & Pemberton Place staff ensuring that allare informed and working in collaboration for the best outcome for thereferred service user.
Main Duties:
Champion the Hospicevalues to provide effective clinical and professional leadership inconjunction with Referrals Hub Manager for the whole team within theReferrals Hub to address the needs of individuals, their families and carers.

To work leading onincoming referrals to the referral's hub providing high standards of clinicaltriage/assessment & specialist palliative care, advice and support toindividuals, relatives and carers, being a role model to other members of theteam & signposting referrals for otherservices as appropriate.
To be responsible forthe leadership and day to day support of staff who work within the ReferralsHub, to facilitate their professional development, nursing practice andensure the highest possible standard of care.
Lead on the day-to-day incoming referrals inclose consultation with the referral's hub team, SCCS team leaders and widerteams throughout SFH as well as external referrers.
To workcollaboratively with the primary care team to provide high quality specialistsupport, advice and symptom control to individuals with specialist complexpalliative care needs and their carers in their primary care setting.
To support the SCCSManagers /Head of Community Services in monitoring and maintaining qualitynursing practice in accordance with national and local standards/guidelines.
Lead on Referral Hub developmentsand respond to all Referral Hub objectives and service delivery goals andopportunities, in line with Hospice strategy.
To be able to workflexibly across a 7-day service as indicated by service need and demand asthe referral hub further develops in line with strategic direction.
Leadership andOperational Management
Tolead and deputize an effective, innovative team.
To identify opportunities for servicedevelopment in liaison with the SCCS Managers & Head of CommunityServices
To attend internal and external MultidisciplinaryTeams for liaison of potential admissions and discharges back to services.
To ensure relevantparties are kept informed regarding the complex management of referrals.
To actively monitor keyperformance indicators regarding referrals for SFH ensuring compliance andreporting any variances to Head of Community Services.
To ensure that theReferrals Hub is always staffed safely according to need and within allocatedresources with accountability for the duty rota in line with SCCS to ensure consistency.
To be responsible forReferrals HR activity including involvement in recruitment and selection ofstaff, orientation and induction of new staff as appropriate
To contribute towardsbudget setting process and where appropriate manage a delegated budgetensuring this is maintained as allocated.
Supervision andco-ordination of Referrals Hub team
To undertakedelegated duties from the Referrals Hub Manager / Head of Community Services inrelation to nursing and clinical issues.
Toco-ordinate/monitor the appraisal system and undertake staff appraisals ifrequested with support from Referrals Hub Manager
Contribute to reports as required forinternal use and for external use regarding referral demand and outcomes.
Ensure and overseeReferral Hub service compliance with all Health and Safety requirements.
Contribute to local,regional and national forums for specialist palliative care services as aReferrals Hub representative.
Develop and maintainworking relationships with other Local Hospices, Acute Trusts, Primary Care,Community Services and Local Authority Services to ensure joint serviceplanning as appropriate.
To ensure that theReferrals Hub provides an excellent learning environment for staff and thoseon placement and ensure that knowledge is shared.
To promote thedevelopment of Referrals Hub nursing and administrative support throughreflective practice and ensure other support mechanisms are in place.
To work alongside the PracticeDevelopment Team to identify and meet the development needs of staff, throughinduction, orientation, appraisal and learning/support following incident.

To assist withfollow- up of recommendations from internal audits.

Person Specification
Qualifications
* Qualified clinical professional (nursing/other ) with current registration and evidence of PIN/Registration.
* Significant clinical senior level experience Band 6/7.
* Degree or equivalent experience in relevant area of work.
* Evidence of knowledge and practical application of team leadership.
* Evidence of Continuing Professional Development.
* In depth knowledge and understanding of the CQC registered service requirements for hospice service delivery.
* Evidence of leadership and management development or willingness to work towards.
Experience
* Mentoring and supervising senior level nursing posts.
* Experience of coordinating an effective duty rota and resourcing a service to meet changing levels of individual needs/complexity.
* Experience of supporting and developing others through an appraisal process; and ability to lead and manage others promoting fairness and teamwork.
* Experience of service redesign and implementation.
* Experience of working with/developing palliative care services.
* Experience of working within the NHS or Local Government and in particular implementing service policies and procedures.
