Job Overview
Exciting opportunity for Children & Young People Case Managers to join the ICB to develop services for young people across Coventry & Warwickshire.
The post holder will play a key role in ensuring delivery of the best possible care within available funding, representing “value for money,” to support the seamless implementation of the National Framework for Children and Young People's Continuing Care (CYPCC). This role includes contributions to the development of the service, training programmes, and facilitating others' learning.
Main Duties of the Job
1. Provide assessment, case management, and review for children with complex healthcare needs subject to Continuing Healthcare or joint funding arrangements, carrying a caseload of the most complex cases.
2. Provide advice, screen referrals, and coordinate assessments in alignment with the National Framework for Continuing Care and local policies, supporting an end-to-end Children and Young People's Continuing Care service.
3. Process and quality assure referrals/screening tools, CYPCC assessments, and ongoing health reviews.
4. Work as part of the ICB Children & Young People’s team to provide a case management approach by collaborating with specialists in learning disability, mental health, physical disability, and end-of-life care.
5. Support and facilitate the transition of young people with care packages moving from children’s services to adult services.
Person Specification
1. Professional Registration currently registered with governing body (e.g., Registered Nurse, Social Worker, or Allied Health Professional).
2. Degree level qualification or equivalent experience and knowledge.
3. Significant experience of working with children and young people with complex and long-term health needs, disabilities, and those with life-limited/palliative or end-of-life care.
4. Experience of working with children & young people with Learning Disabilities, Autism & Mental Health conditions.
5. Sound working knowledge of the National Framework for Children's Continuing Healthcare.
6. Experience of managing and reporting risks.
