Main duties of the job
As an Advanced Nurse/Urgent Care Practitioner you will be confident working autonomously carrying out clinical consultations, providing assessment, diagnosis, treatment and prescribing. To succeed in this role you will need excellent interpersonal, communication and organisational skills.

We are a well established training practice for GP trainees, nursing students and other health professionals. Job Summary: Thepost holder is an experienced nurse or paramedic who, acting within their professionalboundaries, will provide care for the presenting patient from initial historytaking, clinical assessment, diagnosis, treatment and evaluation of care. Theywill demonstrate safe, clinical decision-making and expert care, includingassessment and diagnostic skills,for patients within the general practice. The post holder will demonstratecritical thinking in the clinical decision-making process. They will workcollaboratively with the general practiceteam to meet the needs of patients, supporting the delivery of policy andprocedures, within the remit of telephone triage and acute minor illnessmanagement.

Clinical Practice
Assess, diagnose, plan, implementand evaluate treatment/interventions and carefor patients presenting with an undifferentiated diagnosis
Clinically examine and assesspatient needs from a physiological and psychologicalperspective, and plan clinical care accordingly
Assess, diagnosis, plan,implement and evaluate interventions/treatments for patients with complex needs
Proactively identify, diagnoseand manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
Diagnose and manage both acuteand chronic conditions, integrating both drug- andnon-drug-based treatment methods into a management plan
Prescribe and review medicationfor therapeutic effectiveness, appropriate to patientneeds and in accordance with evidence-based practice and national and practice protocols, and within scope ofpractice

Person Specification
Qualifications
* Current registered Nurse (NMC) or registered Paramedic (HPC)
* Evidence of continued professional development.
* ENP/ ECP Minor Injuries and Minor Illness course Level 6 .
* Long term conditions management qualification
* Relevant MSc or BSc
* Experience as an autonomous practitioner within a minor injury/illness centre or mobile service.
* Experience of triage/initial assessment.
