We are excited to recruit a locum consultant within the Department of Medicine for the Elderly (DME). The successful candidate will work with existing substantive Consultants to provide a comprehensive service for elderly patients with plans to span across both the acute and community settings. The post holder will support the delivery of an Acute Frailty specialist service as part of the Needs Based Care Model implemented across the Trust.
This is an opportunity for an enthusiastic and energetic geriatrician to play a key role across multi-disciplinary teams focused on developing responsiveness and capacity in a range of services for older people in the Luton & Dunstable area.
The unit prides itself on providing comprehensive, high-quality Acute Medical Care of the Elderly through daily consultant input and good multidisciplinary team working.
The on-call physician in medicine for the elderly is the site medical coordinator in the event of a major disaster.
The post holder will contribute to the collective aim of Bedfordshire Hospitals, enabling older people to live as independently as possible, empowering them to take control of their condition to prevent avoidable deterioration and reduce the need for acute hospital intervention.
Main duties of the job
Maintenance of the highest clinical standards in the management of all patients presenting to the Hospital, including optimising safe and timely discharge and supporting patient flow.
Share responsibilities with colleagues in the management of all complex patients referred to the Trust, including senior medical input for frail elderly patients in the Emergency Assessment Unit and Emergency Department.
Work collaboratively with partner organisations to support the development of integrated care, provide specialist guidance to community MDT teams, avoid unnecessary hospital admissions, and manage patients via rapid access clinic.
Actively participate in departmental and Trust matters concerning Clinical Governance and audit.
Take responsibility for and actively participate in continuing professional development (CPD).

Job responsibilities
The appointee will join the Elderly Medicine department in the delivery of clinical excellence within Elderly Medicine.
There are 5 base DME wards at Luton and Dunstable University Hospital, including a Frailty unit and Rehab ward, with a total of 134 inpatients. Luton is leading the way for frailty as a key element of an Integrated Care System (ICS) as part of the BMLK STP.
The Trust is developing its DME strategy and working across Bedfordshire with social care partners, Primary care networks, community and mental health service providers to improve patient care across the acute interface and deliver better patient outcomes.
Consultants joining the organisation are given dedicated time in their job plans for new service development and to pursue their sub-specialty interest. We encourage applications from new Consultants or those more experienced who are keen to have a fresh challenge.
The post-holder and other consultant colleagues will be expected to cover each other's absences on annual or study leave.
Person Specification
Qualifications
* Medical Degree
* oMD or equivalent Higher Degree or evidence of relevant research
Registration
* oFull GMC Registration with a licence to practice
* oEntry onto the GMC Specialist Register: CCT (or within 6 months), CESR
Clinical experience
* oExcellent all-round experience of DGH work
* oEvidence of thorough and broad training and experience in General Medicine/hyper acute, acute and rehabilitation aspects of stroke Medicine
* oAble to take full and independent responsibility for clinical work and delivering service without direct supervision.
* oSub Specialty interest in neuro-intervention and/or newer modalities of imaging to detect early ischemic strokes
Clinical Effectiveness
* oUnderstanding of clinical governance and ability to demonstrate ongoing CPD
* oExperience of guideline appraisal
* oParticipation in relevant local audits
* oAbility to interpret and apply clinical research
* oParticipation in relevant national audits
Teaching
* oKnowledge and ability to teach all grades of trainees and medical students
* oHas attended Teaching the Teachers or similar course
Clinical Governance
* oAble to demonstrate a broad understanding of the principles and elements of clinical governance.

* oResidence within 10 miles by road or up to thirty minutes travelling time of the hospital for purposes of on-call emergencies.
