We currently have an exciting opportunity within our care navigator service, the role will involve shift work across 7 days between 8am to 8pm. You will be part of a flexible team of 3 Care Navigators both on the inpatient ward and alongside the acute mental health community team and receive support and supervision by a Senior Nurse. You will be working with Service Users to establish their needs and helping to develop personalized community care packages to enable their recovery. Build strong professional relationships with other stakeholders. This position is an ideal opportunity to work in a supportive environment with an extended multi-disciplinary team. You will require excellent communication skills to build rapport with patients, colleagues and services in the local community. The successful candidate will have a customer focused approach, be highly organised and be able to deliver to agreed timelines and demonstrate the ability to quickly and efficiently identify and complete the tasks independently. Previous experience of working within the NHS is desirable but not essential as full training will be provided. Car owner / driver essential Main duties of the job To support the planning, co-ordination and transfer of service users across the care pathways. To work in collaboration with the team to support service users as experts in their own care, promoting the principles of recovery, choice and personalisation, which value and empower the service user. To work in collaboration with other parts of the pathway (e.g. home treatment; establishing care packages, statutory AMHP work) to deliver seamless and timely packages of care, geared to the needs of the service user. To develop the role of Care Navigator in order to embed the Care programme approach within the Mental Health Division.
