An opportunity has risen for an NMP to work within the Milton Keynes Community Mental Health Hub.
The post holder will participate and lead positively on promoting and developing a consistent, safe and high-quality service and will actively input in the continuing development and delivery of this innovative and creative service. Excellent communication and motivation skills are essential, as is the ability to promote strong leadership. The Community Mental Health Hub has a strong track record of providing both external and internal training for our staff in line with the service development and needs.
• To act as a supplementary and independent non-medical nurse prescriber (NMP)
• NMP will be competent to take a patient history, undertake clinical assessment including physical health and to diagnose for the purpose and suitability of prescribing interventions and to initiate these within agreed prescribing guidelines for NMP staff.
• To maintain a personal caseload of clients as agreed with the service manager
• To consider the holistic needs of service users and identify actions to address these needs within the context of a jointly agreed and negotiated care plan (DIALOG+).
• The NMP will prescribe and review medication for therapeutic effectiveness, appropriate to patients needs and within evidence-based practice, local and national protocols within their scope of practice.
• To provide supervision / advice to partner agency workers / professionals on issues in relation to non-medical nurse prescribing
• To develop strategies for continued professional development as Nurse Prescriber and to ensure that they maintain and upkeep their general awareness, training and knowledge of general mental health prescribing, having involvement in the in-house CPD programme.

• To offer advice and guidance for pharmacological interventions and provide treatment to a caseload depending on role within the team, and will be responsible for monitoring this through supervision.
• Facilitate the effective mental and physical assessment, diagnosing, care planning, prescribing and review of clients
• Generating prescriptions for clients using appropriate prescribing formularies and monitoring and reviewing the client regarding the effectiveness, continued appropriateness and safety of the prescribing intervention.
• Reporting of remedial action in situation of adverse drug reactions via yellow card and incident form
• Ensure that appropriate systems are in place in accordance with local policy
• To utilise research skills for audit and policy development and provide accurate and timely performance data on service activity as required
• Link regularly with the other agencies to facilitate effective communication between NMP clinics, medication clinics, wellbeing clinics and the MDT Teams to ensure a quality services is maintained.
• In partnership with the Service Management, actively contribute to the programme for Clinical Governance / Best Value and take the lead in the absence of the Lead Prescriber (Medical Consultant)
• To assist the Leadership Team Managers in maintaining up to date statistics on the teams performance to ensure that Performance Indicator targets are achieved and accurate data is compiled.
• To be supportive of the multi-professional team and encourage effective team working, implementing and maintaining high professional standards within and outside of the team.
• Responsible for the supervision and appraisal of staff members and be involved in the in-house CPD programme
• The post holder will work autonomously within professional guidelines and the overall framework of the service’s policies and procedures.
Clinical Work

• To undertake all duties within CNWL NHS Foundation Trust policies and in accordance with local and national standards.
• To consider the holistic needs of service users and identify actions to address these needs within the context of a jointly agreed and negotiated care plan (DIALOG+).
• To undertake risk assessment and risk management in line with Trust guidance and to communicate this appropriately to all agencies/individuals involved in the clients care
• To maintain appropriate, accurate and up to date client records as directed by the Service Manager
• Ensure that clinical practice is developed and maintained in line with the latest research and evidence base.
• Comply with the Nursing and Midwifery Council Professional Code of Conduct, relevant legislation, procedures and policies. To attend and contribute effectively to relevant professional forums.
• Assess, plan, implement and review treatment provision within speciality competence.
• To undertake risk assessment and risk management in line with Trust guidance.
• Ensure that agreed care coordination approaches are applied to all clients within the services to include regular assessment of risk.
• To attend review meetings, case conferences, professional meetings and network meetings as appropriate.
• The NMP will prescribe and review medication for therapeutic effectiveness, appropriate to patients needs and within evidence-based practice, local and national protocols within their scope of practice.
