Requirements:
6. Assessment and Planning : Evaluate the medical, social, and emotional needs of patients to develop comprehensive discharge plans.
7. Coordination : Collaborate with healthcare providers, community agencies, and insurance companies to arrange for home care services or transfers to other facilities.
8. Support and Counseling : Provide counseling and emotional support to patients and their families during the discharge process.
9. Education : Educate patients and their families about available resources, support systems, and their rights.
10. Documentation : Maintain accurate records of all interactions and discharge plans in the patient’s medical record.
What we are looking for in a Social Worker:
11. Social Work England registration
12. Eligible to work in the UK
13. Hold a full UK licence
14. Post-qualified experience
15. Degree level or equivalent in Social Work
