We are looking for an ambitious Specialist Community Public Health Nurse (Health Visitor) to join our award-winning service in Somerset. Using your lived experience, you will be able to get involved in service development and share ideas and best practice.
You must be an NMC registered Nurse or Midwife with a SCPHN qualification and an interest in holistically addressing the health needs of children aged 0-5 years and their families.

* The salary for this post is in line with Agenda for Change Band 6, which has a current salary range of £22,403 to £26,977 per annum for 22.5 hours per week, or £29,870 to £35,969 per annum for 30 hours per week.
* Glastonbury (30 hours per week) and Highbridge (22.5 hours per week).

Main duties of the job
Your day-to-day work will involve:
* Lead on the delivery of the Healthy Child Programme as defined within the service specification, identifying packages of care based on the Universal, Targeted, and Specialist models of service delivery.
* Provide leadership and supervision to other members of the Public Health Team, ensuring you delegate appropriately to other team members and work within Somerset Local Authority policies and procedures.
* Take responsibility for the outcomes of the children and collaborate with key partners to address identified public health needs.
* Develop and foster effective professional relationships and networks with families and other key partners.
* Provide families and children 0-5 with a service that promotes their physical, mental, emotional health, and wellbeing.
* Contribute to multi-agency early support assessment of children and families and take on lead professional role where appropriate.

Job responsibilities
1. Responsible for ensuring the co-ordination and delivery of skilled, effective care to promote positive health and respond to identified, individual client and population/community needs.
2. Provide professional and team leadership to manage a defined team delivering care to a geographical caseload/population.
3. Work with vulnerable children and families in line with national and local policies relating to safeguarding children.
4. Work in partnership with other agencies, particularly health services, education, and social care.
5. Identify those children who are vulnerable and/or at risk from harm.
6. Attend professionally related working groups as appropriate.
7. Maintain a working knowledge of public health targets and ensure that practice reflects their objectives.
8. Responsible for care plans/programmes delegated to members of the health visiting team.
9. Work in partnership with families and other agencies to promote and support a healthy lifestyle.
10. Provide safe and competent advice for individual clients receiving care.
11. Investigate and report complaints and incidents at a team level.
12. Offer advice, guidance, and support to children and families.
13. Understand, uphold, and promote the aims of the councils equality, diversity, and inclusion policies.

Person Specification
Experience
* Experience of working across agencies.
* Understanding and experience of the application of research in practice.
* Evidence of child protection experience.
* Experience in identifying and assessing health needs including community health needs.
Qualifications
* Registered Health Visitor Public Health Specialist Practitioner Degree (or equivalent qualification pre-1988).
* Evidence of continued professional development.
* Leadership programme or prepared to undertake a course.
* Nurse Prescriber V100 Qualification or equivalent.
Knowledge and skills
* Demonstrate working knowledge of the Public Health and Preventative Strategy.
* Working knowledge of child protection procedures and policies.
* Ability to organise and deliver education on a 1-1 or group setting.
* Willingness to lead teams Public Health Nurses and Assistants.
* Proven clinical and managerial leadership ability.
* Good listening, communication, and ability to form good working relationships with colleagues and clients.
* Audit and research skills.
