We are seeking a highly skilled nurse, social worker or Occupational Therapist to join our Bed Management team in the role of Complex Case Manager. Main duties of the job The successful candidate will be based at Sceptre Point, Sceptre Way, Walton Summit, and will be responsible for providing strong, visible day to day clinical leadership skills. The post holder will actively case management a cohort of service users with complex needs, co-ordinating and progressing discharges. The post holder will be required to maintain constructive relationships with a range of stakeholders. The Complex Case Manager role will provide clinical support to Flow leads/OAP practitioners to maximise opportunities to enable safe discharge from hospital. The post holder will focus and lead on patients with complex needs/long hospital admission and will work with agencies to progress discharges.
