West Walk Surgery are expanding their Care Navigator/Reception team and have a 20 hour a week position available.
We need cover for the following hours -
Monday 2pm -8pm
Wednesday -
Friday
We also ask that you are able to offer holiday cover where needed.
Main duties of the job
We are looking for a happy. caring person to assist our patients in being directed to the correct clinician for advice/treatment working alongside our GP led KLINIK team.
You will be Care navigating by answering phone calls, dealing with patients face to face, sending messages to patients and answering queries sent through our clinical system KLINIK.
We have an established, friendly Care Navigator team waiting to welcome and train you.

West Walk Surgery is a large friendly practice, consisting of 5 GP Partners, 5 salaried GP's, Mental Health Nurse, Pharmacy team, Paramedics, Nurse Practitioner, Practice Nurses and HCA's, all supported by the Care Navigator and Admin teams.

Job Summary:
1. Toensure that the Practice runs efficiently and that a good service is providedto patients, doctors, colleagues and other agencies.
2. Toparticipate as part of your team. To carry out your duties efficiently andcompetently. To ensure that patients are attended to quickly and in a friendlymanner. To ensure that duties required by Partners or senior staff are carriedout swiftly. To assist in the provision of a pleasant working environment forall staff.
3. Towork at both the Yate and Coalpit Heath branch surgery.
Duties andResponsibilities:
4. Work with our GP led KLINIK team.
5. Makefuture appointments for patients with doctors, nurses and otherclinicians.
6. Navigatepatients to the most suitable person to deal with their problem.
7. Providegeneral information such as results of tests carried out.
8. Registerregular, temporary, immediately necessary or emergency patients using theapproved guidelines.
9. Takevisit requests according to guidelines, ensuring you have obtained the patientsname, address, telephone number and message.
10. Generalreception administration
11. Whenreceiving patient samples, record on computer and take to treatment room.
12. Whenrequired chaperone a doctor when they are examining a patient.
13. Processsick note certificate requests.
14. Amendmentsand deductions
Printamendments and deductions summary and accept the requests.
Private medicalwork
15. Processincoming and outgoing medical requests.
16. Liaisewith outside agencies and patients in connection with medical report requests.
17. Havea good working knowledge of all office equipment and use as required andinstructed.
18. Respondto the call assist system as per the guidelines.
19. Usethe panic button as per the guidelines.
20. Attendpractice meetings as required.

22. In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately
