This is a London Borough of Havering position - full-time 36 hours per week. To supervise the Support, Time and Recovery (STR) and Peer support workers (PSW) Team. To focus on the direct needs of service users. To work across boundaries of care. To provide support, give time and thus promote recovery to clients with more complex needs.
We are looking for a flexible and dynamic individual who enjoys working in a multi-disciplinary team and has the capability to work independently in the community. We would like someone who has experience of working in a community setting, who has worked independently with adult Mental Health clients. Our aim is to focus on client recovery to enable people to live independently and achieve their desired goals.
Main Duties of the Job
1. To supervise, as directed by the Social Inclusion and Resources Manager, the Support, Time and Recovery Team and Peer support workers.
2. To be responsible for a small caseload of service users.
3. To establish supportive and empowering relationships with the service user and carer.
4. To work in isolated situations with service users who may be vulnerable or at risk and also to support team members to do the same.
5. To liaise with other agencies and any other relevant professional, and to direct STR/PS Workers in doing so.
6. To be responsible for monitoring changes, both positive and negative within the lives of those who receive services from the STR/Peer support Team as a whole, and notify the Support Services Manager.
7. To take appropriate action in cases of emergency, and to offer direction to STR and Peer support worker Team in consultation with the Support Services Manager.
8. The post holder would be expected to lead in the set up, facilitation and overseeing of our community groups to encourage social integration.
Our working hours are 9 - 5 Monday - Friday (Ad hoc weekend and bank holiday working). The base can be one of mental health and wellness teams located at either:
1. (South MHWT) Oasis House, 28-30 Gubbins Lane, Harold Wood Essex RM3 0QA
2. (North MHWT) The Petersfield Centre, Petersfield Avenue, Romford Essex RM3 9PB

Job Responsibilities

2. Carry out high quality holistic assessments, using strengths-based, person-centred, community-focused, and multi-agency interventions to bring about positive change through developing effective relationships and to secure the best outcomes for people and carers.
3. Understand the legal and statutory duties of the post as required by the Care Act.
4. Provide a person-centred service which focuses on asset-based interventions to enable people to optimise and maximise their abilities, by using a strengths-based approach.
5. To pro-actively support and work collaboratively with health services to ensure people receive the required health and social care support within the community.
6. Support people to maintain their independence within their homes by utilising support in the local community and the person's personal networks.
7. To be responsible for a small caseload of service users to establish supportive and empowering relationships with service users and carers.
8. Support the appropriate allocation of resources required to maintain independence through the use of Personal Budgets.
9. Contribute to maintaining quality standards, and ensure self-development is maintained.
10. Promote effective communication within the team and with colleagues internal and external of the organisation.
11. Assess and evaluate risk in partnership with service users, carers, and the wider MDT where required.
12. Use knowledge and skills to support appropriate recovery-focused care plans, promoting choice and independence.
13. Assessing and using professional judgment to assess level of need and urgency.
14. Promote and deliver new ways of working and evidence-based practice within the service.
15. Develops and implements reflective practice in all areas of work.
16. Ensure safeguarding issues are raised in line with the Havering policies and procedures.
