An exciting opportunity has arisen to join our multi-disciplinary PCN team. We are a dynamic, forward-thinking PCN team who are passionate about developing and delivering excellent quality local services to meet the needs of our patients. We are all small traditional partner-led GP practices and we try and keep the best features of traditional General Practice while looking to make the most of new opportunities for our patients.
Due to the changes in the ARRS Scheme to introduce General Practitioners, we are looking to expand our team by seeking to employ a newly qualified General Practitioner to work alongside our practices, Primary Care Network and Integrated Neighbourhood Team staff.
Main duties of the job
1. To perform as an autonomous general practitioner working independently and with other healthcare professionals to assess, diagnose and treat the conditions of patients attending within a primary care setting.
2. Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence. To be involved in the provision of education and training to GP ST3s and other staff.
3. To be professionally accountable and responsible for all elements of clinical practice in accordance with the GMC code of conduct ensuring the safety of patients and quality of their care is primary.
4. Providing counselling and health education.
5. Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.
6. Recording clear and contemporaneous consultation notes to agreed standards.
7. Compiling and issuing computer-generated acute and repeat prescriptions.
8. Prescribing in accordance with the ICB prescribing formulary whenever this is clinically appropriate.
9. The post-holder will be required to visit patients during surgery time in an emergency if clinically indicated.
10. The Frailty session will involve working with our care coordinator and social prescribers to provide pro-active care in the community to frail patients, particularly those from deprived areas who have recently been admitted to hospital or who are at risk of being admitted.

Job responsibilities
1. In accordance with the practice rota, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing/authorising repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
2. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
3. Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
4. Screening patients for disease risk factors and early signs of illness.
5. In consultation with patients and in line with current practice disease management protocols, developing care plans for health.

13. The post-holder will undertake a fair share of home visiting sessions per week as allocated. There is a local visiting service so it is anticipated that routine visiting will be minimal. However, it is anticipated that planned visits will be undertaken during the Frailty session as part of anticipatory care planning for this group of patients.
Person Specification
Experience
* Experience of working in a primary care environment.
* Experience of continued professional development.
* Experience of QOF and clinical audit.
* Newly Qualified General Practitioner with under 2 years from qualification.
* Not previously been substantively employed at a GP Practice.
* General understanding of the primary care contract.

Qualifications
* Qualified GP.
