Our service users live in their own homes and have learning disabilities; some also have health diagnoses. You will support their health needs and health promotion to empower people with learning disabilities.
Each service user is staffed by a passionate and dedicated team. This is an excellent opportunity to practice within the challenging yet rewarding field of community-based learning disability services while working alongside a multi-disciplinary team.
This Team Leader post is a full-time Monday to Friday role. We have an on-call system in place to support the healthcare assistants in their roles. The on-call system is 24 hours per day, 365 days per year, and is currently shared between the manager and the three team leaders. This post will require you to become part of this on-call rota. This will involve an uplift in salary, depending on how many you do, and if called, you will receive overtime payment for out-of-hours work. As this is a community post, being a driver is essential for this role.
Main Duties of the Job
This role requires you to demonstrate your commitment to work proactively and in partnership with people with learning disabilities and use your professional knowledge and experience to support individuals within the community while maintaining effective links with mainstream services, working in partnership with colleagues and supporting equal access to healthcare.
You will provide support and sometimes work directly with people with learning disabilities in their homes, including working with their families, healthcare assistants, other healthcare professionals, and social workers. You will be a dedicated Team Leader for at least two services.
To ensure all practice is based upon the principles of Valuing People: rights, independence, choice, and inclusion. You will work with the individual and their carers in the formulation, implementation, and review of a person-centred care support plan and risk assessments that outline the support the individual requires to continue to live successfully within the community.
As a Team Leader, you will also support Healthcare Assistants who work daily with our service users, providing clinical expertise, advice, and guidance, ensuring the specific needs of the service users are being met. You will carry out training and supervision, manage sickness return and sickness management, cover shifts via the rostering system in place, and help with medication management and support staff with various issues that arise.
