Assistant Practitioner - Falls Prevention and Management Therapy

This is a fantastic opportunity for an assistant practitioner to join the team as we expand our service. You will be involved in the implementation of a falls service across CTM.
This service will provide falls advice, assessment, and intervention when needed at local venues. We are developing a service that looks at both prevention and intervention - expanding on some exciting work already going on in CTM and making this available to our whole community.
The team will work with an individual and their carer to make decisions about the best way to reduce the risk of the person falling, raise awareness of risk factors for falling, and make any necessary changes to their home to prevent falls.
We are creating innovative offers of assessment, advice, and intervention, building strong integrated teams and designing a seamless pathway for our patients.

Main duties of the job
1. Support Allied Health Professions (AHP) namely Physiotherapists (PT) and Occupational Therapists (OT) in the delivery of AHP intervention for patients in their own homes and community, to prevent and manage falls.
2. Work as part of the CTM Falls Prevention and Management Team to undertake specific clinical and administrative tasks, liaise with health colleagues, Local Authorities, and third sector organisations to ensure care for patients within the CTMUHB/Bridgend/RCT/Merthyr County Boroughs as instructed and delegated by the AHP, responding flexibly to the needs of the service.
3. Support the AHP staff by assisting with all modalities of therapeutic care focused toward maintaining the patient within their own home and community and the prevention of avoidable falls and subsequent effects.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Person Specification
Qualifications
* Evidence of good education, to include English, Mathematics GCSE or possess equivalent qualification, or skills knowledge and experience.
* OCN/Agored accredited modules or relevant NVQ level 3.
* Knowledge relating to a range of core AHP activities.
* Knowledge of the factors that can influence the ability to function within patients' own home and community.
* Knowledge of a broad range of health conditions impacting on function relating to falls.
* Successful completion of relevant competency-based training.
Experience
* Experience of developing/delivering education advice to individuals/groups.
* Experience of multi-disciplinary team working.
* Experience of working as HCSW in a medical, rehabilitation, or community setting.
* Experience of falls prevention work.
* Previous experience of NHS, acute clinical, or community setting.
Skills
* Excellent verbal and written communication skills.
* Excellent ICT, computer literacy, and keyboard skills.
* The ability to work independently whilst under appropriate supervision.
* Ability to work with patients with communication barriers/motivational issues.
Personal Qualities
* Ability to work independently and as part of a team.
* Ability to motivate self and others.
* Confident in approaching tasks.
