An exciting and rare opportunity has arisen to join our Eastwood Locality Team within the Health and Social Care Partnership, Adult Services. Working across East Renfrewshire, you will be part of a friendly and supportive multidisciplinary team responding to residents' health and social care concerns and queries. This will involve assessment and case management for local residents, undertaking duty work for the team, managing a caseload, and reviewing individual cases in line with our Supporting People Framework. Ensuring individuals get access to all of the appropriate services available to them, their carers, and families at the right time.
You will have a good understanding of the current health and social care landscape; self-directed support; and the value of outcomes-focused supports for people to enable them to live in their communities. Recognizing the importance of forming effective relationships with other professionals and agencies, including the third sector and community partnerships, is essential in supporting individuals across East Renfrewshire.

This post is considered Regulated Work with Children and Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. It is an offence to apply if you are barred from working with children and protected adults. Additionally, if you have lived or worked in any country outside the UK in the past 10 years, there may be a requirement for an overseas police check.
If you are not currently registered with SSSC, you will commence employment as a Social Work Assistant (Grade 7 - £29,745 to £31,954) until full registration is obtained.
This post requires Scottish Social Services Council (SSSC) registration. You must currently have registration or be eligible to apply from the commencement of employment (you must complete registration within the first 6 months of employment).
