Job Description
The NDIS Case Manager role is a key role in supporting NDIS participants who receive service delivery from All About Living. This full-time role is based at our Aspley office with some flexibility to work from home. The key tasks of the role include:
* Assisting with NDIS referral, intake, assessments and reviews
* Develop individualised care plans
* Conduct Intake meetings with participants, liaise with providers, schedule and confirm bookings forservices
* Update alerts and requirements in the CRM system to facilitate Services and Support Worker communication
* Liaise and work collaboratively with Services staff and provide assistance with complex cases
* Update changes to care needs and inform relevant stakeholders as appropriate
* Ensure accurate record and documents are maintained
Desired Skills and Experience
* Qualifications in Health and/or Human Services (preferably in aged care, disability care, community care and/or community mental health)
* Experience in conducting assessments, reviews, intake and develop individualised care plans
* Intermediate computer skills with ability to work withExcel and other MS Office programs, with Alayacare experience desirable
* Demonstrated positive experience of interacting with NDIS participants
* Current Driver’s License, own registered and fully insured vehicle
* Satisfactory National Police Check
* Well-developed organisational and time management skills with the ability to prioritise and manage workloads to meet targets and deadlines
* Self-directed with the ability to work both independently and within a team environment
* Excellent communication and interpersonal skills
