Job summary:
Primary Care Networks (PCNs) form a key building block of the NHS long-term plan. Bringing general practices together to work at scale has been a policy priority for some years for a range of reasons, including improving the ability of practices to recruit and retain staff; to manage financial and estates pressures; to provide a wider range of services to patients and to more easily integrate with the wider health and care system.
Following the release of ARRS GP funding, we are looking for newly qualified (within 2 years) to join Meridian Primary Care Network Ltd on a fixed term salaried contract for 6 months. We will consider applications for between 4 and 6 sessions with a start date as soon as possible.
The post holder will be working as an independent GP across our six practices, and will spend a variable amount of time supporting PCN projects focusing initially on our new Frailty project with the potential to work on other projects in the future. The post holder will also be invited to join our PCN management meetings as appropriate to their role.
Meridian offers flexible working, within surgery hours, thus no evening or weekend working. This role is practice based with community visits; our surgeries include Bourn, Comberton, Harston, Monkfield (Cambourne), Roysia, and Orchard (Melbourn).
Primary responsibilities:
* To work with older people in surgery and out in the community including care homes.
* To manage their own caseload.
* Engage individuals as active partners in their treatment and support.
* To support onward referral to secondary and specialist services where indicated.
* To develop and maintain close relationships with the surgery teams.
* Maintain patient records.
* Undertaking patient consultations and physical examinations.
* Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
* Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
* Screening patients for disease risk factors and early signs of illness.
* Providing general pre- and post-operative care.
* Monitoring and administering medication.
* Assessing and planning treatment requirements.
* Liaising daily with staff including other doctors, non-medical management staff and healthcare professionals.
* Promoting counselling and health education.
* In general, the post-holder will be expected to undertake all the normal duties and responsibilities.
Secondary responsibilities:
* To assertively engage with service users, striving at all times to develop good relationships, becoming a trusted person.
* To develop professional networks, have an understanding of local resources in order to ensure that the service user is able to navigate the full health and social care system.
* To promote awareness of the professional role across the PCN.
* To demonstrate effective communication skills with people in an age-appropriate way and those whose understanding may be impaired.
* To act within the appropriate professional code of conduct at all times.
* To ensure that up-to-date written and electronic records and activity data are maintained in accordance with Professional standards and to provide specialist reports relevant to practice setting.
* A commitment to life-long learning and audit to ensure evidence-based best practice.
* Contributing to evaluation/audit and clinical standard setting within the organisation.
* To keep accurate and timely written and electronic record of each contact, using SystmOne and EMIS.
* To promote and safeguard the welfare of vulnerable adults.
* Attending training and events organised by the Practice or other agencies, where appropriate.
