Community Respiratory/Rehabilitation Assistant

This is an excellent opportunity for an enthusiastic and committed person to join the Community Respiratory Service in Newhaven as a Band 3 Community Respiratory Rehabilitation Assistant. You will be working as part of a dynamic multi-disciplinary team comprising of nurses, physios, occupational therapists, cognitive behavioural therapists, associate practitioners and community rehabilitation assistants.
CRS support chronic respiratory patients in the community. We work to help patients independently manage their lung condition on a day to day basis and to prevent unnecessary hospital admissions. The successful applicant will be responsible for following care plans given by all members of this multidisciplinary team. The role consists of home visits, teaching and assessing skills of self-management. You will also assist in pulmonary rehabilitation provision and clinics. There is also an administrative element to the position.
We are looking for an excellent team player who has outstanding communication skills, good time management and organisational skills. These are all essential requirements of this post.
It is essential that applicants hold a full UK driving licence and have a car available for work, as the successful applicant will be required to undertake home visits independently.
Main Duties of the Job
1. Under the supervision of a qualified practitioner, participate in planned treatment activities (such as spirometry, nebuliser provision, inhaler technique, Home exercise programmes) taking into account the fluctuating needs of the patient.
2. To implement care plans and follow programmes set up by Physiotherapists, Occupational Therapists and Nurses.
3. To observe any changes in a patient's condition, record accurately and report back any changes or concerns to an appropriate clinician.
4. To undertake patients' physical measurements under the direction of a qualified practitioner.
5. To maintain effective documentation in patient records as required by the integrated respiratory service and Sussex community NHS Trust.
6. To undertake further training to gain skills and knowledge in specific clinical areas as needed e.g., spirometry/phlebotomy.
7. To assist the qualified practitioner with home assessments if required.
8. To assess and instruct in the use of prescribed daily living equipment if necessary and practice its safe use with the patient.
9. To report effectively to the team on patients' progress and performance in relation to the patient goals.
10. To assist in the induction, training and education of students, volunteers and other staff members.
11. To participate in audit and evaluation of the service as requested.
12. To provide support for secretaries and/or admin staff in the team ensuring cover during annual leave etc.

Person Specification
Qualifications and/or Professional Registration
* Good standard of education
* NVQ Level III Health Care or equivalent experience
* Teaching module/course
Experience
* Experience of working in a caring environment in health, social or voluntary services
* Experience of working with adults in the community
* Experience with chronic disease advice
Skills and Knowledge
* Ability to work as a team member
* Ability to work unsupervised
* Ability to use initiative and have an empathetic approach
* Willingness to undertake job related training, multidisciplinary competencies and NVQs
