Post: 3 x 37.00 hour posts and 1 x 18.50 hour post, based at various locations across the Dudley borough with some home working. You will be working in the Assessment and Independence Team to help prevent hospital admissions. Your primary role will involve working alongside social work colleagues to provide support in assessing vulnerable adults within a hospital or home setting to contribute to the development of individual support plans. These plans enable people to achieve the optimum level of independence, to maintain relationships, and be valued members of their community. You may also be required to undertake responsive assessments for people in crisis in the community. You will need to be able to work under pressure and prioritise your own workload. You will also need to have professional commitment and a high level of knowledge, critical thinking, communication skills and creativity. You will be working 37.00 hours per week and will participate in a set rota which is based over a 7 day period and will include some weekends, some evenings and a set number of bank holidays. Relevant weekend and bank holiday enhancements will be paid. You will need to work flexibly, travelling across the Dudley borough. You will therefore need to hold a valid full driving licence and have the use of a vehicle.
