Are you passionate about improving the health of patients?
If the answer is yes, we have an opportunity for Health Care Support Workers to join a team who deliver brief interventions to patients within our General Practices and other community settings to people who are at risk of developing type 2 diabetes as part of the All Wales Diabetes Prevention Programme (AWDPP) across Cwm Taf Morgannwg.
The post holder will be expected to undertake appropriate training to use the appropriate software and have excellent communication skills and possess behaviour change and motivational interviewing skills which will enable them to support and motivate patients to make positive lifestyle changes, understanding their risk factors and enable them to reduce and manage their risk of developing type 2 diabetes.
The perfect candidate would have knowledge of the Cwm Taf Morgannwg UHB demographics. Candidates for this post must be able to demonstrate at interview that they have strong communication skills and be prepared to provide real-life instances of where they demonstrate the skills outlined in the person specification attached.
Apackage of training and support will be offered to the suitable candidate and you will joining a small supportive team who are flexible and work autonomously.
Main duties of the job
Advise on healthy living options and behaviours for individuals, in their context
Help individuals to develop an 'action plan' to change behaviour and sustain those behaviours, leading to more healthy choices and actions
Support individual's 'action plan' over an agreed time through practical help
Help individuals to identify and access local services
Keep agreed records and monitor progress
Have a good knowledge of local services which can support clients in improving their health and well-being and reduce their diabetes risk
Support patients to access local services that will improve their health and well-being
Support individuals to recognise and change harmful behaviours
Offer one to one intervention to patients to support and motivate them to improve their health
To accurately record clinical data
This post is fixed term for 18 months due to - meet the needs of the service

4. Level 3 Diploma in Clinical Healthcare Support, or equivalent qualification or experience If deemed appropriate willing to work towards a City and Guilds Level 3 Certificate for Health Trainers Willing to undertake further training appropriate for the role, Motivational interviewing and brief intervention training. Willingness to undertake Venepuncture Training Understand and utilise the principles of behaviour change and motivation
5.
