An opportunity has arisen for a highly skilled, enthusiastic and motivated physiotherapist to join the management team of Integrated Care across Greater Nottingham.
FDIDT are an in-reach multidisciplinary team made up of AHPs, Nurses and Assistant Practitioners. This Nottingham CityCare Partnership service provides innovative, successful and patient focussed outcomes for those that present in the emergency and urgent pre admission areas at Nottingham University Hospitals QMC site.
Your role within the busy and dynamic team will be to provide a comprehensive holistic assessment of need with the aim of preventing avoidable hospital admissions.
The successful candidate will be able to work within the MDT as an autonomous practitioner, undertake comprehensive assessments, and complete profession specific assessment and evaluation as required, to identify the appropriate discharge plans for adults with a wide variety of complex physical and mental health needs.

Main duties of the job
Clinical

Job Purpose
To work as an autonomous practitioner, you will undertake comprehensive assessments to identify the appropriate service for the clients referred.
To carry out profession specific assessment and evaluation as required.

To be responsible for continuing the on-going development of high standards of clinical practice with evidence-based focus within the service.

Dimensions
This post has no budgetary responsibility; however, the post holder is responsible for demonstrating cost consciousness.
You will be required to work primarily with the Queens Medical Centre but with potential towork across different bases and community settings which may include clients own home,residential, nursing homes or hospital.
You may be responsible for a designated caseload of clients.
