Are you looking for a new and challenging role as part of a dynamic team?
We are looking for a motivated and enthusiastic Advanced Pharmacist to join the Hospital at Home Team in Aberdeen City. We have up to 37 hours available for the right candidate(s) with both part-time and full-time applications being considered. Please include your preferred hours/days within your application form.
Hospital at Home provides hospital level acute medical care within patients' own homes. While the service is currently open daily from 8am – 8.15pm, it is not anticipated that weekend/unsocial working would be required for this post.
The post is offered on a one-year secondment/fixed term basis. If you are currently employed by the NHS on a permanent contract, you must have a discussion with your manager and have a secondment agreed in principle before submitting your application. Please include that this discussion has taken place within your application.
As an Advanced Pharmacist, you will be responsible for coordinating and delivering a clinical pharmacy service for patients within the Aberdeen City Hospital at Home Service. This will include an independent prescribing role; provision of expert advice and training to the Hospital at Home multi-disciplinary team; and ensuring cost-effective, evidence-based prescribing. You will also provide day-to-day management to the team of 3 pharmacy technicians.
As the first Pharmacist within the service, this new post offers an exciting opportunity to influence and develop the role and procedures whilst actively supporting and improving the health of people accessing services. Excellent communication and interpersonal skills are essential.
You will take a lead role in the team advising on and delivering high quality, evidence-based pharmaceutical care and advice in line with national clinical guidance. Strengthening links with local GP practices, community pharmacies, and hospital services will be key to your success. You will be an experienced pharmacist with good clinical knowledge and the ability to work effectively as part of a team in a multidisciplinary environment. You will also be required to take an active role in the supervision and training of staff and students across the team, to act as a role model, and to participate in audit and research.
Candidates must have a degree in Pharmacy and be on the practising register of the General Pharmaceutical Council (GPhC). Candidates must also be an Independent Prescriber (IP) and be willing to develop in this role. Completion of a clinical diploma/NES Advanced Competency Framework (or equivalent knowledge demonstrable through experience) is also essential. Applicants should ensure they specify their qualifications/experience in their application form in order to meet the essential shortlisting criteria (detailed in the person specification) especially where the required knowledge is gained through experience rather than a formal qualification or competency framework.
Ability and willingness to travel throughout the Aberdeen City area is essential for this post. The successful candidate must therefore own or have access to a car for work purposes and hold a full UK driving licence.
