You will be working with some of the most marginalised patients in society as they encounter Police Custody as part of their criminal justice journey.
MAIN DUTIES:
An exciting opportunity has arisen for a Band 6 Practitioner with a criminal justice interest. The Sussex Health & Justice Service is based in Police Custody across the locality with this post based in Worthing. The role includes providing high-quality assessments and supporting people coming into contact with the criminal justice service.
Key tasks include:
* Arranging appropriate support for service users and liaising with professionals within the criminal justice system.
* Completion of assessments within custody.
* Checking health records to aid triage and identify vulnerabilities.
* Onward referrals to services required to meet individual’s needs.
* Ensuring all relevant agencies are informed of needs and risks for individuals.
* Point of contact for professionals based in Police Custody.
* Work as part of a team with the freedom and ability to work independently.

Responsibilities include:
* Take responsibility for specialist assessment (within service specification) in Liaison and Diversion in conjunction with service user and carer, helping to promote self-determination, choice and autonomy.
* Provide written reports for Crown or Magistrates Courts or attend and give verbal evidence as required.
* Participate in the setting of quality standards, including the auditing, monitoring, and reviewing of practice in line with current clinical guidance practice and policy.
* Assess patient's individual care needs, agree, and refer on to appropriate agencies.
* Complete and contribute, where appropriate, to robust risk assessments and risk management plans.
* Maintain an effective reporting system by observing and reporting verbally and in writing on patient condition.
* Utilise agreed outcome measures to review the effectiveness of process, as part of Health and Justice Service.
* Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
* Assist in ensuring that the aims and objectives of the service (as set by the Team Leader) are fulfilled and to identify factors which may inhibit these from being achieved.
* Assist in the implementation of evidence-based practice and participate in research and other projects as required.
