To work within the Community Nursing Team to provide care and support for patients in the community. The registered Nursing Associate will work as part of the Nursing workforce supporting Registered Nurses to deliver high quality, person centred, compassionate care to patients in their homes or care homes, as delegated by the registered nurse or registered health care professional. They will be part of the multi-disciplinary/integrated team delivering planned care to patients working within the standards of proficiency outlined by the Nursing and Midwifery Council and in accordance with HRCH policies and protocols, with access to advice and supervision as required.
To carry out clinical duties and be prepared to undertake clinical skills update as per needs of the service and specific patient after training and competence assessment, with support from an identified mentor for example, blood pressure, pulse and temperature recording, urinalysis, blood glucose monitoring, wound dressings, and non-complex assessments and reviews.
This will involve working independently but as part of the team covering geographical areas in Hounslow.
Main duties of the job
We are looking for an enthusiastic Registered Nurse Associate to provide safe and effective care as a valued member of the community nursing service as delegated by the registered nurse. The Registered Nursing Associate will work autonomously within the community using their enhanced clinical skills.
You will be supported with your development in practice to deliver safe and effective care.

Person Specification
Qualifications
* GCSEs Grade A-C in Maths and English or key skills level 2 in Maths and English
* Successful completion of the nursing associate training and registration with the NMC
* Demonstrable ability to study at Level 6 Nursing apprenticeship degree programme and commit to completing any professional development trainings.
* Qualifications and Credit Framework Level 3 in Care or equivalent in terms of both academic attainment and previous care experience
Experience
* Essential induction training and experience - care certificate or equivalent
* Previous experience of working within a health / care setting
* Understanding of the scope of the role of the Nursing Associate in context of the team and the organisation, and how the role may contribute to service development
* Completion of an HCA development programme
Skills and Abilities
* Able to demonstrate a commitment to the HRCH values (Care, Respect, Communication)
* Ability to move between sites working across health and social care as required by the needs of the service
* Understanding of and application of evidence base practice
Knowledge
* Understanding of basic physiology, eg. normal vital signs, fluid balance, nutritional requirements etc
* Evidence of involvement in support / development of less experienced staff
Personal Attributes
* Car driver (full UK licence) with use of vehicle or means of transporting self for work purposes
* Courteous, respectful and helpful at all times
