Job summary This is an exciting opportunity to work within our dynamic and innovative Acute Frailty service delivering quality, holistic care to our older patients with frailty in the Emergency Department (ED) at Musgrove Park Hospital. The post-holder will demonstrate a high level of autonomy, clinical decision making and expert skill to manage and care for older frail patients with acute / chronic illness. It will require the post-holder to advocate for the needs of our older population and to be part of the multi-disciplinary team that adds real value to the patient experience and outcomes whilst delivering an efficient and effective service for the Trust and wider healthcare community. At least 80% of the role will be clinical, with an aim to impact on: Identifying patients with frailty in the Emergency Department Holistic assessment of these patients and patient focused care Ensuring appropriate interventions are scheduled and completed at the right time and place in the individual's healthcare journey Raising awareness and knowledge of everyone working in ED of the needs of patients with frailty Main duties of the job They will work with the ED team to identify those needing a comprehensive geriatric assessment (CGA) and coordinate input required. The post-holder will support decision making about the intervention plan and appropriate pathway of care. This will include complex discharge of patients directly from ED and identifying the patients for whom the Acute Frailty Unit is the most appropriate destination.
