Job summary Are you an experienced pharmacist looking to enhance your career in a specialist clinical role in an acute hospital Trust? Working as a pivotal member of the High Cost Drugs pharmacy team you will be the lead clinical pharmacist supporting the homecare service in specialist areas where high cost drugs and biosimilars are the focus. Your areas of clinical speciality will include rheumatology and dermatology and you will support the team of clinicians and nurse specialists in providing high cost drugs to patients through a homecare service. You will also have the opportunity to utilise your prescribing skills within the specialty working alongside clinicians in supporting the management of these long term conditions. Due to service innovation and Trust investment this new role has been created to better enhance service delivery. You will be based at Ipswich Hospital, however cross site working to Colchester is required. Your role will be key in ensuring high quality and cost effective medicines are provided to patients as part of the medicines optimisation strategy. You will maintain close links with NHS England and ICB in the provision of high cost drugs via Homecare delivery service. ESNEFT is about to embark on a full site wide implementation of the EPIC electronic patient record which includes electronic prescribing and administration as well as pharmacy stock control system. About our Pharmacy Service Main duties of the job Fulfil role as the lead clinical pharmacist for the homecare services in the provision of medicines via homecare to patients at East Suffolk and North Essex NHS Foundation Trust. Optimise the processes involved from reviewing a funding application to initiation of treatment for patients. Contribute to the development of the pharmacy homecare service. Co-ordinate processes for the introduction of homecare services for new medicines, including the development and implementation of local policies, in liaison with the pharmacy homecare team and homecare providers Provide clinical leadership and expertise for pharmacy homecare consultations and homecare queries Monitor and evaluate key performance indicators for homecare services and investigate incidents related to homecare services. Support, develop and monitor the training and education of all pharmacy staff working in homecare services and users of Blueteq Attend and actively participate in service review meetings between multidisciplinary clinical teams and homecare providers in relation to specific homecare service level agreements, and provide specialist pharmacy input to ensure compliance with professional, clinical and operational standards.
